Form 1040 U.S. Individual Income Tax Return 2004. Summary: This is a Form 1040 that was completed by Jane M. Stone for the final joint return filed by her after her husband's death. Across the top of the form is written in order “DECEASED,” “JOHN S. STONE” (the husband's name), and “FEBRUARY 28, 2004” (the date of his death). The computer generated address label listing both John and Jane is affixed to the “Label Here” area. Under the “Your social security number” field is 765-00-4321, and under the “Spouse's social security number” field is 123-00-4567. In the section labeled Third Party Designee, the line reads “Do you want to allow another person to discuss this return with the Internal Revenue Service (see page 58)?” and the “No” checkbox is marked. Under the Sign Here section, the “Your signature” field contains Jane M. Stone, the “Date” field contains 4/1/05, the “Your occupation” field contains Engineer and the “Spouse's signature. If a joint return, both must sign” field contains Filing as surviving spouse.
