Quick Reference Chart for Form 5500, Schedules and Attachments--continued. Summary: This list represents the table of reference as to what schedules need to be completed with Form 5500 dependent upon
what type of
pension plan the taxpayer participates in. The information presented lists the situations for Schedules I, P, R, S.S.A., T
and the attachment of an
accountant's report.
Schedule I (Financial Information--Small Plan)

Large Pension Plan: Not required.

Small Pension Plan: Must complete.

Large Welfare Plan: Not required.

Small Welfare Plan: Must complete. *2

*2 Unfunded, fully insured and combination unfunded/insured welfare plans covering fewer than 100 participants at the beginning
of the plan
year that meet the requirements of 29 C.F.R. 2520.104-20 are exempt from filing an annual report. (See Who Must File on page
3.) Such a plan with 100
or more participants must file an annual report, but is exempt under 29 C.F.R. 2520.104-44 from the accountant's report requirement
and completing
Schedule H, but MUST complete Schedule G, Part III, to report any nonexempt transactions. See What To File on page 7.
